How to Provide Proof of Other Coverage
Team Members who wish to waive the mandatory Extended Health and/or Dental coverage must provide proof of other coverage. To fulfill this requirement, please complete the following steps.
If you previously waived coverage and did not provide “Proof of Other Coverage”, you will need to submit the form below showing that you still have coverage from another insurance company. If you do NOT complete the form, you will automatically be enrolled in Michaels Extended Health and Dental Coverage at the 2026-2027 premiums. Payroll deductions will begin with the first check in July.
Please be prepared to provide the name of your other insurance carrier and your policy number.
Step 1: Complete the Proof of Other Coverage Form
- Linked below in English and French is the Proof of Other Coverage Form.
- Use the “View” button to open the form and fill out the appropriate fields.
- Then, use the “Save” option in the top right corner to save your completed form (PDF) to your device.
If you have trouble filling out the form in your web browser, you can use the “Download” button to save the form (PDF) to your device fill it out that way, or print a paper copy.
Step 2: Upload the Completed Form to Workday
If you chose to print the form and complete the paper copy, you will need to scan or take a picture of the form to upload in Workday.
- Log in to Workday.
- Upload and submit your completed Proof of Other Coverage Form.
