Canada Life requires Evidence of Insurability (EOI) for Team Members who wish to make any of the following changes to their benefits:
- Enrol in Medical and/or Dental coverage through Canada Life as a late entrant outside open enrolment or not during a qualifying life event
- Enrol in Optional Life and AD&D Insurance and/or Voluntary Critical Illness Insurance
The EOI requirement applies during initial enrolment, during open enrolment, throughout the plan year, and for qualifying life events.

Identify the Type of EOI Request
A. Late Applicant
(Enrol in Medical and/or Dental coverage through Canada Life as a late entrant outside open enrolment or not during a qualifying life event)
If you are enrolling in benefits for the first time or adding a spouse/dependent without a qualifying life event, complete the following forms:
- Late Applicant Form
- Application for Group Coverage Form
- Evidence of Insurability – Late Applicant Benefits Request (Form M5995)
B. Increasing Coverage Amounts
(Enrol in Optional Life and AD&D Insurance and/or Voluntary Critical Illness Insurance)
If you are increasing Optional Life Insurance or Voluntary Critical Illness coverage, complete:
Team Members must complete the employee sections of these forms before submission.
Submit Forms for Review
- Open a Knowledge Zone ticket.
- Attach all completed and signed EOI forms from Step 1.
- The Benefits team will complete the employer section and send the forms to Canada Life at: groupmed@canadalife.com.
Canada Life Review and Processing
- Please allow up to four (4) weeks for Canada Life to review and process your EOI request.
- If you have not received a status update by mail after four weeks, open a Knowledge Zone ticket for assistance and reference your Knowledge Zone case number from Step 2.

The EAP includes counseling services for all Michaels Team Members:
- Full-time Team Members are eligible to receive up to three (3) face-to-face or televideo counseling sessions per issue at no charge.
- Part-time Team Members are not eligibile for face-to-face counseling sessions, but are eligible to receive confidential counseling by phone at any time.
You can call 24 hours a day for in-the-moment emotional well-being support. Services are free and confidential, and can help with a wide range of issues, including:
- Anxiety
- Relationship support
- Depression
- Stress management
- Work/life balance
- Family issues
- Grief and loss
- Self-esteem and personal development
- Substance misuse and more
Financial Constitution Resources
Sometimes a little help can go a long way. You can get a free 30-minute consultation for each issue you’d like to ask about, from creating a budget to setting long-term goals.

Through the Michaels Canada Employee Assistance Program (EAP), you have unlimited access to Supportiv, an online peer support service that can help you cope with stress, work, family issues, loneliness, anxiety, parenting, motivation, and more.
Supportiv is an online tool that lets you talk through any mental health, emotional, or social struggles in small group chats with people who can relate. Discussions are anonymous and are guided and safeguarded in real time by professional facilitators. Supportiv is anonymous and available 24/7.
Supportiv is available to all Michaels Team Members and their household members.

Tuition Assistance
Michaels offers tuition assistance to help you pay a portion of the cost for your college tuition or language programs for professional and personal development.
Most Team Members are eligible to receive up to $2,000 per year in assistance. To apply, you must meet all criteria outlined in the Tuition Reimbursement Policy (also available in French), and complete the necessary form. For more information and complete eligibility requirements, contact Team Member Services. You may also log in to the Knowledge Zone, click Human Resources and then Tuition Assistance.

Adoption Assistance
Interested in adoption? You may be eligible for adoption assistance! Michaels will reimburse you up to $5,000 per adoption to help cover adoption fees, attorney fees, court costs, travel expenses, medical fees (for the child) and temporary foster care. To apply, you must meet all criteria outlined in the policy (available in French and English) and complete the necessary form within six months of the finalization of the adoption.
